Health History
Before your first session you will be asked to fill out a Client Information Sheet. Please use the link, under Client Forms, to print and fill out the form before your first session.
To provide proper therapy at every subsequent session it is the client’s responsibility to always inform the therapist of new and existing health conditions.

Arrival Time & First Time Clients
Please arrive for your session a few minutes prior to your scheduled appointment to allow time to discuss any new changes.
New clients should allow up to 15 minutes before the massage to fillout the intake form and discuss any health issues with the therapist.

LATENESS
All scheduled sessions begin and end on time. Appointment times have been arranged specifically for you. If you are late for your massage appointment you will be provided as much time as available within the scheduled time slot in order to accommodate others whose appointments follow yours. You will, however, still be charged the full amount for your scheduled session.

Confirmations
Appointments will be confirmed the day before the scheduled massage.

Payment
Cash, checks, and credit cards, are accepted at this time. Please make checks payable to Kevin Burns.

$25.00 fee for returned checks.

Physician Referral
A physician referral is generally not required in order to receive massage therapy. After speaking with your therapist and reviewing health history information a determination will be made prior to your session if a doctor’s note is required.

*Massage therapy is not a substitute for medical examinations and/or diagnosis. It is recommended that you see a physician for any physical ailment you have.

Cancellations
Cancellations are sometimes unavoidable. Please call within 24 hours notice to allow ample time for filling that appointment.

Cancellations require a 24 hour notice. In an emergency any notice you give is appreciated to avoid the cancellation fee, due to something out of your control or mine. It is understood that there may be a time when you need to reschedule your appointment due to illness or emergency.

GIFT CERTIFICATES
 If using a Gift Certificate it must be turned in at the time of the services otherwise cash or check will be required.If you made an appointment with Kevin Burns, CMT and do not show-up for your appointment the certificate will be forfeited.
